A 31-year-old woman presented with a rare condition causing uncontrollable, involuntary laughter without any accompanying joy, a phenomenon doctors termed 'joyless laughter.' This condition, which began in childhood, involved a distressing sensation before each episode and difficulty speaking, swallowing, or breathing during the fits. Initially misdiagnosed by her parents as deliberate behavior, the attacks, which occurred multiple times daily and lasted for minutes, gradually lessened in intensity over time. Medical examinations, including MRI and EEG, initially showed no abnormalities, but a video analysis of her laughter led doctors to suspect a seizure disorder known as gelastic seizures.

The case is significant because it highlights a rare neurological condition, hypothalamic hamartoma, presenting with unusual symptoms and a remarkably benign progression, offering new insights into the disorder.
