Romen Dixon has been in a mental health ward since he was arrested on May 8 after he allegedly took a knife from Woolworths at Toowoomba's Grand Central Shopping Centre and threatened customers.

Mr Dixon had been hearing voices "instructing him to kill", according to a psychiatrist's report that was read out in court.

"He remained convinced that his parents and strangers were involved in a conspiracy," Magistrate Veena Goverdhan read.

The psychiatrist wrote that if three men had not physically restrained the teenager, Mr Dixon said he would have "seriously harmed" the man to get a confession.

The teenager was charged with intending to cause grievous bodily harm, going armed as to cause fear, and stealing.

Mr Dixon's father, Richard Dixon, said he was not aware his son was experiencing such a severe mental health crisis.

"He's been struggling for a long time now. He just needs help, and that's what we're here for," he said.

He and his wife drove 2,000 kilometres from where they live in north-west Queensland to Toowoomba to attend court this week.

On Friday afternoon, Magistrate Goverdhan granted the teenager bail to return to north-west Queensland and live with his parents.

At the time of the alleged offence, he had been living independently in Toowoomba.

Under bail conditions, Mr Dixon is required to attend the local hospital or general practice fortnightly for antipsychotic medication and to comply with the conditions and directions of his doctors and other treating mental health professionals.

If he missed an injection or failed to comply, his treatment team would be required to notify a member of the Queensland Police Service.

Police prosecutor Amanda Stephens opposed bail, citing concerns for the safety of Mr Dixon's family.
